In response to the evolving Coronavirus Disease 2019 (COVID-19) national emergency, we are following the latest CDC recommendations.
CORONAVIRUS ALERT:
Do you have any symptoms of fever, cough, shortness of breath?
Have you traveled to high-risk areas in the last 14 days OR if you have been in close contact with someone who has traveled to these areas?
Have you been in close contact with someone who has been diagnosed with COVID-19 (within 6 feet of that person for an extended time or exposed to their cough or sneeze)?
PLEASE NOTIFY YOUR PHYSICIAN IMMEDIATELY - PRIOR TO COMING TO CLINIC!
NEW VISITOR GUIDELINE:
In response to COVID-19, only 1 visitor is allowed per patient if the patient requires assistance.
No one under the age of 18 is permitted to accompany the patient.
Visitors who have flu-like symptoms or recent travel to high-risk areas or close contact with a person diagnosed with COVID-19 are not permitted to visit our office.
